Nurse Practitioner - Primary Care

Provide high quality nursing and medical services to patients and their families, emphasizing health promotion and disease prevention, as well as the diagnosis and management of acute and chronic diseases. The core Success Factors include: Instill Trust and Value Differences Patient and Community Focus and Collaborate RESPONSIBILITIES Evaluate sick, urgent care, transitional care and primary care patients Perform annual physical exams as needed Educate patients on maintaining proper health Evaluate, make recommendations, co-manage and treat patients' medical needs Participate in an interdisciplinary team providing care and coordination of our patients, ensuring the highest level of care MINIMUM QUALIFICATIONS Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
