Job Overview Full‑time Family Nurse Practitioner for South Mountain Health Center, Phoenix, AZ. Day shift. 4-10's on location. Salary range: MSN $115K-$120K+ DOE, DNP $129K. Arizona FNP License required. FNP Experience Preferred; NextGen Experience is a PLUS. Language differential pay: $3/hour for Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili or ASL. Responsibilities Performs independently or as part of a treatment team. Conducts physicals/check‑ups, evaluations, diagnostics, treats illnesses, orders laboratory tests, provides general and preventative care, prescribes and monitors medications, and conducts individual treatment and case consultation. Educates patients about preventative care, wellness activities and prescribed treatments. Provides leadership and cohesion within the clinic environment, and may provide direct supervision to other professional and/or non‑professional medical staff and students. Participates in program evaluation procedures and professional record keeping, and makes referrals appropriately for specialty and behavioural health care. Provides direct patient services not less than 85% of the work time. Employs therapeutic interventions appropriate for identified treatment issues. Seeks appropriate clinical supervision/support when necessary to ensure the provision of effective treatment. Establishes appropriate patient files that conform to agency guidelines, best professional practice and State licensing regulations. Qualifications Nurse Practitioner AZ License. Master’s degree or Doctorate in nursing (Family Nurse Practitioner). Valid BLS certification. Valid DEA license and Board Certification (ANCC). Valid Arizona driver’s license, 21 years of age with minimum 4 years driving experience, and compliance with Terros Health’s driving policy. Valid Arizona Fingerprint Clearance card or application for Level 1 within 7 working days of assuming role. Pass Drug Screen, TB Test and Background Check. Proficient using computer‑based systems including electronic health records (NextGen preferred), Microsoft Outlook and other Microsoft Office products.
