Join an established team of 2 other physicians and 3 Apps Call structure: 1:6 Practice setting: Majority of time will be in the outpatient clinic, ability to round on patients as needed in local hospital(s) and occasionallya home visit might be necessary. Employed position within a nonprofit health system Board Certified or Board Eligible in Internal Medicine required with Geriatrics fellowship preferred Eligible for state medical licenseand DEA registration About PACE: The Program for All Inclusive Care (PACE) is an alternative to nursing home care and - through an interdisciplinary care team of physicians, nurses, social workers, therapists, and aides - coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Aged 55 years old or older. Live in a PACE providers service area. Certified by the state as meeting the need for the nursing home level of care. Able to live safely in the community with the help of PACE services at the time you enroll. While there are no financial eligibility requirements, PACE programs are covered by Medicare and Medicaid for eligible individuals. PACE services are also available on a private pay basis. Advanced Life Support preferred. American Heart Association or American Red Cross accepted. Education: Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
