YOUR ROLE The Community Nurse Coordinator is an integral member of the Community Options team, supporting Connecticut residents, their families, and community partners, including access agencies, healthcare providers, and other service providers. This role focuses on helping older adults and individuals with disabilities access the supports and services needed to remain safely in their homes or successfully transition back into community living.
Many of these services are provided through Medicaid Home and Community-Based Services (HCBS) programs and waivers, which allow Connecticut to offer community-based alternatives to institutional care by providing flexible supports that meet each individual's unique needs.
This position will provide nursing support and clinical oversight for the Money Follows the Person (MFP) Demonstration, a federal initiative designed to help individuals transition from institutional settings, such as nursing facilities, back into the community with appropriate long-term services and supports. The Community Nurse Coordinator will play an important role in supporting successful transitions, promoting independence, and ensuring individuals receive services in the most integrated setting possible.

POSITION HIGHLIGHTS
- SHIFT: first shift | full time | 40 hours per week
- SCHEDULE : 8:00AM - 4:30PM
- LOCATIONS : 55 Farmington Avenue, Hartford, CT

- Provide consultation and guidance to providers, participants, and their families;
- Coordinate authorization and delivery of home and community-based services;
- Evaluate the cost effectiveness and clinical appropriateness of medical, social, and behavioral health services; and,
- Ensure compliance with applicable state and federal regulations.

EXAMPLES OF DUTIES
- Provide consultation and case management to access agencies, health care providers, clients and family members;
- Coordinates authorization of delivery of services with access agencies;
- Evaluates cost and clinical appropriateness of medical, social and behavioral health aspects of care;
- Conducts health care case reviews for quality and care management;
- Reviews and approves or denies prior authorization requests for services;
- Serves as a resource to clients and health insurance companies regarding rights and responsibilities;
- Serves as liaison with internal agency contacts and outside providers regarding program policies and procedures;
- Completes health screen assessment and determines level of care eligibility;
- Evaluates care plan to ensure compliance with cost caps and federal regulations;
- Reviews, evaluates and authorizes requests for medical services and equipment for clients;
- Investigates client complaints;
- May conduct on site quality assessment and/or compliance review;
- May represent the department in fair hearings related to medical necessity determination of nursing facility level of care;
- May make recommendations for policy changes;
- Performs related duties as required.
- Considerable knowledge of principles and practices of nursing care administration for variety of specialties with varied settings;
- Knowledge of
- relevant state and federal laws, statutes and regulations;
- relevant agency policy and procedures;

- Incumbent in this class must possess and retain a license as a Registered Nurse in Connecticut OR hold a Multistate or Compact Registered Nurse license.
- Incumbent in this class may be required to travel.
- Incumbents in this class may be required to possess a valid Motor Vehicle Operator's license.
