Responsibilities:

• Develop and implement effective social services programs and services that meet the needs of our diverse resident population

• Collaborate with the interdisciplinary team to ensure seamless care coordination and delivery

• Provide emotional support and counseling to residents and family members

• Develop and maintain relationships with community resources, organizations, and referral sources

• Recruit, train, and supervise social services staff and volunteers

• Participate in quality improvement initiatives to ensure continuous improvement and excellence in social services

• Maintain accurate and timely records and reports
