CHFNP is seeking a licensed Family Nurse Practitioner to join our team on a per diem basis, providing primary and preventive care to patients in Palm Coast and Daytona, Florida. This role supports CHFNP's mission of delivering accessible, high-quality care to underserved, homebound, and elderly patient populations across home, and assisted living facility (ALF) . Assignments and shifts are scheduled flexibly based on patient volume and coverage needs in each service area.

Roles & Responsibilities

Clinical Care

• Conduct comprehensive history and physical examinations for patients in the home, ALF, and/or office

setting as assigned.

• Evaluate, diagnose, and manage acute and chronic conditions within FNP scope of practice.

• Develop and implement individualized treatment and care plans, including medication management.

• Order, perform, and interpret diagnostic tests (labs, EKGs, Holter monitoring, point-of-care testing) as clinically indicated.

• Coordinate care with collaborating specialists (cardiology, urology, and other referral partners) as

needed.

• Charts must be completed within 24-48 hours.
• Deliver care in compassionate care with empathy, kindness and respect.
• Background check is required as part of hiring process.
