Description

• Conduct thorough evaluations for patients in both inpatient and outpatient settings, creating tailored treatment plans
• Implement effective physical therapy interventions targeting mobility, strength, balance, and functional capabilities
• Work closely with interdisciplinary teams including nurses, physicians, and other rehabilitative professionals to enhance patient outcomes
• Guide and educate patients and their families on therapeutic objectives, discharge processes, and essential at-home exercises
• Maintain accurate documentation of assessments, treatments, and patient progress in line with facility standards
• Engage in team rounds and comprehensive care conferences to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or its equivalent from an accredited institution

Certifications

• Valid state license as a Physical Therapist
• Current CPR certification preferred
• Additional certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory

Skills

• Prior professional experience is strongly preferred
• Ability to address the needs of patients with complex medical and mobility challenges
