Field-Based Community Health Advocate
Saint Luke's Hospital of Kansas City Transplant Clinic–Wichita
Saint Luke’s Health System is seeking a Community Health Worker to serve the North Region Primary Care Clinics and surrounding areas. The WHP CHW will travel across the coverage area to support at‑risk patients, coach them in managing their health, and help navigate the health care and social service systems. The role involves home visits, attending medical appointments, connecting patients with resources, and documenting all interactions in a timely, accurate manner. #J-18808-Ljbffr Saint Luke's Health System
