Develop and manage individualized plans of care. Deliver skilled therapy focused on ADLs, safety, fall prevention, and independence. Address home safety, adaptive equipment, and functional training. Communicate directly with physicians and the interdisciplinary team. Educate patients and caregivers. Complete documentation in Kinnser supported by AI tools. Serve as the primary OT voice in patient care planning. Requirements Current California Occupational Therapist license. Strong clinical judgment, organization, and time-management skills. Home health experience is preferred, but strong outpatient, inpatient, or rehab OTs are encouraged to apply.
