Description

• Conduct assessments of patients in both inpatient and outpatient settings, creating customized treatment plans to address their individual needs.
• Implement physical therapy interventions tailored to enhance mobility, strength, balance, and functional abilities.
• Work alongside nurses, physicians, case managers, and other rehabilitation specialists to ensure the best outcomes for our patients.
• Guide and educate patients and their families on therapeutic goals, discharge planning, and at-home exercises.
• Maintain detailed records of evaluations, treatments, and patient progress according to facility and regulatory standards.
• Engage in interdisciplinary team rounds and care conferences, enhancing coordinated care delivery.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred.
• Any specialized certifications (e.g., neurological, geriatric) are considered advantageous but not mandatory.

Skills

• Previous experience in a clinical setting is highly preferred.
