JOB SUMMARY

The Resident Care Coordinator (RCC) supports the clinical leadership team by coordinating resident care services, supporting clinical staffing processes, monitoring care needs, and assisting with medication and service plan oversight. This role serves as a key liaison between residents, families, clinical leadership, and community partners while ensuring compliance with state regulations and Longevity Senior Living standards. The RCC is not a department head role, but requires strong leadership presence, clinical knowledge, organization, and the ability to influence and coordinate across teams.

DUTIES AND RESPONSIBILITIES

Support hiring, onboarding, orientation, and training of care team in coordination with the Executive Director (ED) and Director of Health & Wellness (DHW). Assist with scheduling care team members using Schedule Pop; ensure schedules are posted timely and meet state and company requirements. Monitor staffing coverage and notify leadership of gaps, trends, or concerns. Maintain survey-ready scheduling documentation as required. Monitor required training completion and skills checklists for care team staff using Relias. Identify and communicate training or compliance gaps to the DHW and ED. Serve as a point of contact for resident and family concerns during scheduled work hours, escalating issues as appropriate. Respond to resident requests and coordinate services to support quality of life. Act as a liaison with community providers. Support invoice review and communication related to clinical and care-related expenses through Yooz, as applicable. Monitor resident care needs and promptly notify the DHW, Director of Nursing (if applicable), or healthcare providers of any change in condition. Participate in daily stand-up meetings and report resident issues, incidents, or trends documented in ALIS. Assist with incident reporting, follow-up, and documentation in ALIS, as required. Support quality assurance audits and provide findings to leadership. Assist with new resident move-ins by ensuring required documentation, physician orders, and care information are in place. Communicate resident needs and preferences to caregiving and dining teams, including allergies and special diets prior to first meal service. Participate in an on-call rotation as assigned. While on call, triage resident care concerns, support staffing needs, and escalates issues appropriately. Monitor employee hours and staffing patterns to support adherence to labor budgets. Identify and communicate overtime risks or staffing inefficiencies to leadership. Support review of departmental labor and supply usage as directed.

QUALIFICATIONS / EXPERIENCE

Recent history of experience in assisted living and memory care setting. Care manager experience in assisted living and memory care setting. Strong customer service, communication, and interpersonal skills. Ability to build trust and collaborate with leadership and frontline staff. Highly organized with strong follow-through and sense of urgency. Comfortable making decisions and directing others, including those who are not direct reports (this is not a managerial role). Able to perform frequent lifting of less than, or equal to, 50 pounds.

EDUCATION AND EXPERIENCE

18 years of age. High school graduate or equivalent GED. Currently registered or certified with the state as a Certified Nursing Assistant in good standing (Preferred). Currently certified and willing to maintain CPR Certification. #J-18808-Ljbffr The Reserve at Oswego
