Key Responsibilities Provide effective patient care using approved therapy techniques and procedures Evaluate patient condition and establish measurable treatment goals Develop and implement individualized treatment plans Supervise and coordinate care with PTAs and HHAs Document patient progress, updates, and treatment notes accurately and on timeEducate patients and families on exercises, mobility, and use of adaptive equipment Evaluate home environments and recommend modifications to improve safety and independence Ensure all services meet HIPAA standards for confidentiality Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care Qualifications Active Physical Therapist (PT) license in MN Minimum 1 year of PT experience preferred Current TB test (within 12 months) and CPR certification Valid driver’s license and reliable transportation Available Opportunities We offer full-time, part-time, and per diem roles in AZ, CA, CT, FL, IL, IN, MA, MN, NV, and OH. #J-18808-Ljbffr NIGHTINGALE HOME HEALTHCARE OF MINNESOTA INC
