Are you a nurse who sees the person, not just the patient? Do you believe that the end of life should be met with dignity, comfort, and unwavering support? Are you looking for a role where your expertise directly impacts the quality of life for families in North Austin?

The Role As a Hospice RN Care Manager, you will be building trusting relationships in the homes of those who need us most. You will provide comprehensive, compassionate care, ensuring every patient's final days are filled with comfort and quality of life.
You will play a vital role in assessing, planning, and implementing individualized care plans for multiple patients in varied locations, while providing the support and education that families desperately need.
Why CareFor Is Different We strive to provide the highest quality, personalized hospice care to seniors and their families in Central Texas, ensuring they can live where they choose to live, with grace and dignity.

That is why we pride ourselves on a supportive management team that is deeply invested in your professional growth. We offer a culture of early and often feedback, where we address needs head-on with respect and view every challenge as a learning opportunity.
What You Will Do (Your Impact)
- Evaluation: Conduct initial and ongoing comprehensive assessments of the terminal disease's impact on patients' physical, functional, psychosocial, and environmental needs. You will listen, observe, and document findings to develop care plans that truly reflect the patient's story.
- Care Planning: Implement and document personalized care plans to ensure quality and continuity. You will make necessary revisions based on patient needs and outcomes, ensuring care evolves as the journey progresses.
- Education: Be a trusted consultant and educator for patients and families. Explain complex disease processes, teach self-care techniques, guide end-of-life care, and address dietary needs. You will also provide training for other staff as needed.
- Care Coordination: Determine the scope and frequency of services needed based on the patient's condition. You will coordinate seamlessly with our interdisciplinary team to prioritize care needs and ensure everyone is on the same page.
- Preventive Care: Initiate appropriate preventive and rehabilitative nursing procedures, applying strict infection control practices to prevent contamination and transmission of disease.
- Clinical Documentation: Prepare clinical and progress notes that reflect progress toward established goals. You will ensure compliance with third-party payor requirements while maintaining the highest standards of patient care documentation.
- Communication: Maintain effective, empathetic communication with patients, families, team members, and other healthcare professionals. You will keep physicians informed of changes in patient needs and outcomes.
- Patient Care Manage a planned weekly patient visit schedule while remaining available to assist with urgent patient or family needs during business hours when necessary.
- Professional Growth: Participate in hospice-sponsored in-service programs, maintain your professional growth, and contribute to the hospice's Quality Assessment and Performance Improvement (QAPI) program.
What You Will Bring, Do, and Own
- Compassion: You are a helper, energized by assisting others, and you operate with genuine kindness.
- Critical Thinking: You possess strong assessment and critical thinking skills, able to make quick, informed decisions in dynamic environments.
- Communication: You communicate clearly and empathetically with families, caregivers, physicians, and interdisciplinary colleagues to ensure seamless transitions and continuity of care.
- Independence & Teamwork: You are able to work independently in the field while remaining a collaborative, positive member of our interdisciplinary team.
- Reliability: You are dependable, loyal, and ready to commute throughout the South Austin area.
Who You Are You're a nurse who thrives on one-on-one care. You want to make a real difference in the lives of others. You have a passion for hospice or palliative care, and you believe that the end of life deserves the same level of care and attention as the beginning.
What You Will Need To Have
- Current RN License: Active in Texas and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact.
- Experience: 2 years of nursing experience preferred (1 year required). Experience in hospice or palliative care is highly valued.
- Transportation: A valid driver's license, insurance, and reliable transportation to and around the North Austin area (including Pflugerville, Round Rock, Cedar Park, Leander and the surrounding communities).
- Mindset: Maturity and the ability to deal effectively with the emotional and physical demands of the job.
- Skills: Ability to carry out directions, read, and write with precision.
What You Will Get
- A Passionate Team: Join a group that believes the employee experience directly impacts the customer experience.
- Support: We encourage a culture of early and often feedback, addressing issues and needs head-on with respect.
- Growth: Opportunities for professional development and career advancement.
- Competitive Pay: Market-competitive compensation that reflects your skills and dedication.
