Key Responsibilities:

• Assess patient health status and monitor ongoing medical and behavioral needs to ensure well-being.

• Develop and implement personalized care plans in collaboration with healthcare providers to meet individual patient needs.

• Maintain accurate, detailed clinical documentation and patient records compliant with healthcare regulations.
• Monitor patient progress, report changes immediately, and adjust care plans accordingly.
• Ensure adherence to all safety protocols and healthcare policies to foster a safe and compliant environment.

Required Skills & Qualifications:

• Active Registered Nurse (RN) license in New Hampshire.
• Associate's degree (ADN) or Bachelor's degree (BSN) in Nursing.
• 1-3 years of nursing experience preferred, especially in community health, outpatient, long-term care, or home health settings.
• Strong clinical assessment skills and the ability to coordinate care effectively.
• Knowledge of chronic disease management and patient education techniques.
• Excellent communication, organizational, and teamwork skills.
