We are seeking a full-time 40-hour Diabetes Program Coordinator to support our Internal Medicine suite Monday through Friday from 8:00-5:00pm. The schedule consists of 4 days in Haverhill and 1 day at our Andover location.

Summary: Responsible for overseeing the successful planning and completion of educational programs for patient care.

Essential Functions:

• Maintain accreditation for the program.
• Conduct individual and group patient education sessions; maintain the schedule of patient appointments.
• Coordinate reporting for audits and implement recommendations as a result of the audits.
• Maintain records pertaining to program activities.
• Purchase or approve the purchase of equipment and supplies needed by the staff and program.
• Schedule program work, oversee daily operations, coordinate the activities of the program, and set priorities for managing the program.
• Manage marketing and outreach to the community.
• Prepare or assist in the preparation of grant requests and funding continuation from outside sponsors.

Qualifications:

Education: Bachelor's Degree Related Field of Study required.

Can this role accept experience in lieu of a degree? Yes.

Licenses and Credentials: Registered Nurse [RN - State License] - Generic - HR Only preferred.

Basic Life Support [BLS Certification] - Various Issuers preferred.

Certified Diabetes Care and Education Specialist [CDE/CDCES] - Certification Board for Diabetes Care and Education (CBDCE) preferred.

Experience: Health Education &/or Literacy Experience 3-5 years required.
