Job Details

Seeking an experienced RN to deliver exceptional home health care services in a field-based setting. This full-time, 13-week contract is ideal for a nurse who thrives on autonomy, meaningful patient interactions, and the ability to positively impact lives in a home environment.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM

Responsibilities:

• Complete thorough OASIS assessments and develop tailored care plans for each patient
• Deliver a full range of skilled nursing services, such as wound care, medication administration, IV therapy, and chronic disease management in patients' homes
• Monitor patient status, respond to changes in condition, and communicate promptly with the interdisciplinary team and physicians
• Educate patients and their families about diagnoses, medications, treatment goals, and strategies for disease management
• Ensure all documentation is accurate and submitted in a timely manner using Home Care Home Base (HCHB)
• Coordinate care with physical/occupational therapists, medical social workers, and home health aides
• Attend case conferences, staff meetings, and continuing education as necessary
• Meet all regulatory and agency standards related to documentation and patient care

Requirements:

• Active RN license in good standing for the practicing state
• At least one year of recent experience in home health nursing (field visits preferred)
• Skilled in using Home Care Home Base (HCHB) electronic documentation
• Demonstrated proficiency with OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
