Social Worker/Care Companion Pottstown, PA Carease Health - PA Carease Health - PA We are seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team at Carease Health - PA . As a Social Worker/Care Companion , you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. About the Role As a key member of our team, you will be responsible for: Visiting designated facilities in accordance with a schedule set with your supervisor Conducting assessments of residents to identify emotional, cognitive, and psychosocial needs Implementing short-term, solution-focused therapy and supportive interventions to address immediate concerns Documenting visit notes and maintaining detailed records of interactions and interventions after each visit Collaborating with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP Requirements Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills 1 year of experience Why Join Carease Health?
