Responsibilities Provide skilled nursing care to medically complex patients, primarily children, in a personalized, low‑stress environment. Administer medications and treatments based on individualized plans. Educate and support families in home‑care techniques. Observe and report changes in patient condition and document visits using the EMR system. Act as a trusted part of a long‑term care team focused on safety, comfort, and continuity. Furnish services, treatments and diagnostic and preventive procedures as assigned per the plan of care. Prepare clinical and progress notes. Assist physicians, intermediate care providers and RN in performing specialized procedures. Prepare equipment and materials for treatments observing aseptic technique. Assist patients in learning appropriate self‑care techniques. Understand and adhere to established policies and procedures. Follow and implement the nursing care plan for each patient as approved by the RN/physician. Initiate preventive and rehabilitative nursing procedures as appropriate. Observe signs and symptoms and report to physician and RN. Teach and counsel patients and family regarding nursing care needs and related problems. Evaluate effectiveness of the LPN's nursing service to the patient and family in collaboration with the RN. Maintain accurate and complete records of observations, treatments and care. Participate in medical record audit as assigned. Attend staff meetings, patient care conferences and in‑services as scheduled. Takes on‑call duty, nights, weekends and holidays as assigned. Submit any changes in schedule to appropriate scheduling position in their office on a daily basis or as applicable by EMR workflow. Participate in patient care conferences to discuss the need for involvement of other members of the health team. Observes aseptic technique and standard precautions. Accepts, writes and executes orders; orders are cosigned by RN where applicable. Ensures that the patient/family is involved in implementing the plan of care. Assists in reviewing, evaluating and updating the patient plan of care as appropriate. Provides appropriate information regarding patient to other agencies or individuals involved in patient's care as needed and as directed by the RN. Observes confidentiality and safeguards all patient related information (PHI) in accordance with HIPAA requirements. Completes documentation and paperwork in a timely manner as per Agency policy. Immediately reports/documents to Nursing Supervisor/RN any patient incidents, variances or complaints. Demonstrates competent performance of technical skills according to established procedures before performing new skills. Participates in peer review, utilization review and QAPI activities as assigned. Provides effective communication to patient/family, team members, and other health care professionals. Understands and adheres to established state, federal and/or accreditation policies/procedures. Maintains acceptable attendance status, per Company policy. Reports all incomplete work assignments to Nursing Supervisor/RN. Maintains clean and neat work environment. Demonstrates sound judgment, critical thinking and decision making. Qualifications Active LPN license. Graduate of an approved school of practical nursing and currently licensed in the state(s) in which practicing. One year experience in nursing (preferred). CPR certification (IV certification by state mandate, if required). Excellent verbal and written communication skills. Physical ability to perform the described duties.
