Job Description - Licensed Practical Nurse (Part time 2nd Shift) (260003Q6) Organization Agency Contact Name and Information: Lori Finch; View email address on click.appcast.io or View phone number on click.appcast.io Unposting Date Ongoing Work Location Southwest Developmental Center, 4399 East Bauman Lane, Batavia, OH 45103 Primary Location United States of America – OHIO – Clermont County – Batavia Job Duties Works under direction of Registered Nurse (RN) and requires substantial knowledge of skilled licensed practical nursing procedures to provide care for residents. Responsibilities include: Administer medication and perform treatments prescribed by physicians, including controlled narcotics and non‑controlled medications orally, rectally or via injection (intramuscular and subcutaneous). Conduct self‑medication programs, provide instructions to families for pass medications. Maintain supplies in medication rooms, reorder as needed. Perform skilled practical nursing procedures such as sterile dressings, oxygen administration, catheter insertion, gastrostomy tube reinsertion, suctioning, irrigations, enemas, douches, specimen collection. Monitor health and safety environment, report and remediate problems. Assist in emergencies: first aid, CPR, psychotropic emergencies. Access emergency boxes, check medications from pharmacy deliveries, maintain records. Deliver medication to cottage med carts. Participate in shift change reports, rounds, observe and document resident responses and conditions, maintain incident and medication reports. Provide general nursing care and support with personal hygiene, grooming, eating, toileting, mobility, behavior. Transcribe and implement physician orders, begin AIR forms. Attend in‑services, workshops, meetings and seminars. Coordinate with Residential Care Supervisor and RN on health care and nursing care plans. Assist in employee orientation, gather information for IPPs under RN direction. Complete required paperwork: quarterly summaries, IPPs, medical appointments, PO sheets, MARS, chart audits. Assist with recreational activities and hospital coverage as directed. Verify physician orders, MARs, and conduct monthly PO and MAR changes. Mandatory overtime is required. Qualifications License to practice nursing as a Licensed Practical Nurse issued by the Ohio Board of Nursing; MDS certification issued by ANNAC. Job Skills: Nursing, Attention to Detail. Unusual Working Conditions May work weekends with rotating days off; may be exposed to unpredictable patient behavior, illness and/or infectious diseases.
