Responsibilities

• Conduct OT evaluations for residents and develop individualized treatment plans to address functional goals
• Implement therapy to improve activities of daily living, mobility, balance, and safety with transfers
• Document progress, reevaluate goals, and adjust plans; participate in interdisciplinary rounds
• Educate residents, families, and caregivers on home programs and carryover strategies
• Maintain accurate documentation and ensure compliance with regulatory requirements
• Collaborate with the care team on discharge planning and transitions as appropriate

Requirements

• Active California OT license
• Degree in Occupational Therapy from an accredited program (OTD or MOT acceptable)
• Ability to work in a skilled nursing/long-term care setting
• Strong clinical documentation and communication skills
• Capacity to meet guaranteed 35 hours per week on a day shift
• 13-week travel assignment commitment starting 08/25/2026
