507 - Case Manager-Care Services

Job Summary Morris Heights Health Center (MHHC) is seeking an enthusiastic individual with knowledge of HIV and case management. The candidate will perform intense case management and outreach services for HIV positive patients, including home visits, patient escorts for appointments, modified directly observed therapy, referrals, outreach, tabling events, recruitment, and health education. The role requires strong writing, analytical skills, and monthly reporting to ensure program goals are met in accordance with policies and protocols. Essential Duties And Responsibilities Perform phone and home visit monitoring to ensure viral load suppression and engagement in care. Provide treatment adherence support, risk and harm reduction counseling. Develop and maintain a referral network of community‑based organizations, SROs, and social services that provide a wide variety of services for impacted patients. Conduct street outreach, tabling events, and recruit patients from local businesses, schools, hospitals, and clinics. Provide newly diagnosed and HIC positive patients with prevention counseling and referral to Positive Life Workshop. Conduct referrals and escort patients to needed services. Coordinate with the data team and manager to support quality assurance activities. Participate in multidisciplinary team conferences, including case presentations. Participate in quality improvement activities. Maintain required documentation of all services in the patients' electronic medical record and Eshare. Complete timely reports as directed by the manager. Perform other duties as assigned. Qualifications Associate's degree preferred (with experience); Bachelor's degree preferred. Minimum of two years of experience working with HIV positive clients preferred. Ability to navigate an electronic health record system. Strong verbal and written communication skills. Strong knowledge of New York‑based community resources for persons living with HIV. Proficiency in Word and Excel. Ability to thrive in a fast‑paced environment, think strategically, and exercise superior judgment. Bi‑lingual; Spanish speaking preferred. #J-18808-Ljbffr Morris Heights Health Center
