RN Sepsis Coordinator

Job Summary and Qualifications The Sepsis Coordinator is a health care professional who works in collaboration with medical and hospital staff to provide multidisciplinary care to septic patients. The Coordinator provides clinical, professional education and expertise for patients, families, physicians and staff members. Scope of practice includes coordinating timely, evidence-based care for the septic patient to ensure optimal patient care management and outcomes. The Coordinator will collect and analyze data surrounding sepsis patients to assist with documentation, coding, clinical care and outcomes. Facilitates development and refinement of sepsis policy and practice to become the standard of care within the facility, while participating in the quality improvement process. Applies good verbal and nonverbal skills to achieve service goals of customer service and satisfaction. Collaborates with all members of the interdisciplinary team to achieve productive movement of best practices within the organization. Maintains active involvement in nursing and hospital committees as necessary. What you will do in this role Review lactate reports Screen the ER tracker and all nursing units to identify possible septic patients and consult with the nurse to ensure proper treatment Round daily in ED and respond to all possible Code Sepsis alerts, providing assistance to nurses and physicians as needed with implementing (and/or educating about) bundle elements Screen for patients who had sepsis but no alert was activated or unable to attend alert (after hours) to ensure all bundle elements were implemented Monitor SPOT data (inpatient sepsis alert system) Submit sepsis coding review questions to HIM service center as needed Print charge nurse shift report to keep track throughout the week as a cross reference to ensure all septic patients are captured Follow up with staff and physicians when bundle elements were not implemented or were not implemented timely to determine root cause and provide education as needed Perform and document findings of chart review on sepsis mortalities Prepare agenda for and lead multidisciplinary sepsis committee meetings Prepare compliance reports from individual chart review Perform chart review with ED physician, intensivists, hospitalist and staff physicians Review division reports and data in Crimson Attend division sepsis calls Qualifications (RN) Registered Nurse Associate Degree Basic Cardiac Life Support must be obtained within 30 days of employment start date Benefits Comprehensive benefits for medical, prescription drug, dental, vision, behavioral health and telemedicine services Wellbeing support, including free counseling and referral services Time away from work programs for paid time off, paid family leave, long- and short-term disability coverage and leaves of absence Savings and retirement resources, including a 401(k) Plan with a 100% match on 3% to 9% of pay (based on years of service), Employee Stock Purchase Plan, flexible spending accounts, preferred banking partnerships, retirement readiness tools, rollover support and financial wellbeing counseling Education support through tuition assistance, student loan assistance, certification support, dependent scholarships and a partnership with Galen College of Nursing Additional benefits for fertility and family building, adoption assistance, life insurance, supplemental health protection plans, auto and home insurance, legal counseling, identity theft protection and consumer discounts We are an equal opportunity employer.
