Prestige Care of Fairfield is a leading provider of subacute rehabilitation and skilled nursing care in the community. We are dedicated to improving the quality of life for our residents by delivering compassionate, evidence-based therapy services in a supportive and collaborative environment.

We are seeking a licensed Physical Therapist (PT) to join our in-house rehabilitation team. In this role, you will provide high-quality, individualized therapy services to residents in our subacute rehabilitation unit, helping them achieve their highest level of function and independence. Flexible scheduling is available, with both full-time and per diem opportunities.

• Evaluate, plan, and implement individualized physical therapy programs for residents

• Provide therapeutic interventions including mobility training, balance exercises, strengthening, and gait training

• Monitor and document resident progress, updating care plans as appropriate

• Collaborate with physicians, nursing staff, occupational therapy, and speech therapy to ensure coordinated care

• Educate residents and families on exercises, adaptive techniques, and safe mobility

• Participate in team meetings and quality improvement initiatives

• Maintain compliance with federal, state, and facility regulations

   

• Licensed Physical Therapist in the state of Iowa

• Minimum of 1 year of experience in a skilled nursing or subacute rehabilitation setting preferred (new graduates welcome)

• Strong clinical reasoning, communication, and documentation skills

• Ability to work independently and as part of a multidisciplinary team

• CPR certification required or willingness to obtain

   

• Per Diem: Flexible scheduling based on facility needs
