Requirements Conduct comprehensive psychiatric evaluations and regular follow-up visits for residents in skilled nursing and post-acute care environments. Develop and adjust personalized treatment plans that meet each patient’s unique mental health needs. Prescribe, monitor, and manage psychotropic medications, ensuring safety and therapeutic effectiveness. Work collaboratively with an interdisciplinary care team —including physicians, nurses, psychologists, and social workers—to coordinate holistic care. Provide therapeutic guidance, counseling, and education for patients and families to support treatment adherence and understanding. Maintain accurate and compliant documentation in the electronic health record (EHR) system. Participate in team meetings, case conferences, and quality improvement initiatives to promote best clinical practices. Qualifications Active New York State Nurse Practitioner license with Psychiatric-Mental Health certification (PMHNP) .
