Physical Therapist (PT) – Full-Time
Civita Care Center at Salmon Brook | Glastonbury, CT
$45–$50/hour | Full-Time | Benefits

Post Rehab is seeking a Physical Therapist (PT) to join the rehabilitation team at Civita Care Center at Salmon Brook. This full-time opportunity is ideal for a clinician who enjoys helping residents restore mobility, build strength, and achieve greater functional independence in a skilled nursing and post-acute care setting.

What You'll Do

• Perform physical therapy evaluations and develop individualized plans of care
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and functional mobility
• Establish resident-centered goals and monitor progress throughout the rehabilitation process
• Modify treatment approaches based on clinical findings and resident response
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies
• Complete accurate and timely clinical documentation
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team

Qualifications

• Active Connecticut Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical judgment, communication, and teamwork skills
• Skilled nursing or rehabilitation experience preferred, but not required
• New graduates are welcome to apply
