JOB TITLE: County Health and Wellness Coordinator REPORTS TO: Hospital Administration

JOB SUMMARY:

The County Health and Wellness Coordinator is a full-time, benefits-eligible, exempt, grant-funded position based in Woodville, Texas, with countywide travel. The position was established under the Rural Texas Strong Initiative 1 (Part 1) award, Make Rural Texans Healthy Again. The incumbent provides county-level leadership for community-based prevention, wellness, and nutrition programs targeting chronic disease, particularly obesity and diabetes. The position emphasizes outreach, health education, care navigation, produce-access activities, and follow-up for residents who experience barriers to care. The existing clinical team remains responsible for direct clinical counseling. This position does not replace dietitian or Community Health Worker roles that may be funded under a companion award. This position should have an overall understanding of all grants associated with the Rural Texas Strong program.

ESSENTIAL TASKS/DUTIES:

Other tasks will be assigned) Plan, implement, and monitor Initiative 1 project activities in accordance with the approved work plan, budget, and Hospital District policies. Establish the program within the first ninety (90) days of employment, including outreach calendars, partner schedules, and required tracking tools. Organize and operate pop-up produce markets, cooking demonstrations, community education sessions, and health-fair activities in Tyler County. Coordinate with the Tyler County Nutrition Center for produce storage, classroom use, wellness-check days, and related community programming. Identify and engage priority populations, including adults with obesity, prediabetes, or diabetes; residents age 65 and older; low-income, uninsured, or underinsured persons; and residents living outside Woodville. Provide navigation assistance so participants can access clinic visits, nutrition classes, produce distribution, and other approved project services. Coordinate participant transportation assistance and in-kind or grocery-restricted incentives in accordance with grant rules; maintain complete issuance and trip logs. Collaborate with outpatient clinic staff so community activities support individual and small-group obesity and diabetes counseling. Assist residents, as appropriate, with information about Medicare, Medicaid, and other coverage or community resources that reduce barriers to preventive care. Maintain required grant documentation, including counts of persons served, event participation, produce distribution, transportation, and incentives. Prepare quarterly participation reports and assist administration with progress reviews against project targets of 730 obesity-related intervention encounters and 250 diabetes management or intervention encounters. Participate in sustainability planning during the final six months of funded operations to identify activities the Hospital District can continue after the grant period. Represent the Hospital District in a professional manner at community sites and maintain effective working relationships with internal staff and external partners. Have core knowledge of all Rural Texas Strong grant initiatives. Perform other duties as assigned that are directly related to the approved grant scope of work. Qualifications Bachelor’s degree from an accredited institution in public health, health education, nursing, social work, healthcare administration, sociology, community health, or a closely related field; or an equivalent combination of education and progressively responsible experience. Two (2) years of experience in community health, rural outreach, patient navigation, health education, or wellness/nutrition programming. Valid Texas Class C driver license and acceptable driving record. Reliable personal transportation for frequent in-county travel. Ability to pass a criminal background check and other pre-employment screenings required by the Hospital District. Preferred: Experience serving rural East Texas or similarly situated communities. Preferred: Experience with diabetes prevention, obesity intervention, senior outreach, or food-access programs. Preferred: Community Health Worker (CHW) certification or eligibility to obtain certification. Preferred: Experience with grant-funded programs, including recordkeeping and reporting. Preferred: Bilingual English/Spanish ability. Knowledge of chronic-disease prevention principles, particularly obesity and diabetes, and of rural access barriers including transportation, poverty, and limited local services. Skill in public speaking, community education, one-to-one resident engagement, organizing events, maintaining logs, and preparing simple quantitative reports. Ability to work independently in the field with limited daily supervision and to collaborate with clinicians, nutrition-center staff, and community partners. Ability to handle confidential information in accordance with Hospital District policy and applicable privacy laws, and to communicate clearly, orally and in writing. Work is performed in an office, clinic, nutrition-center, and community-event setting. The position requires frequent local travel throughout Tyler County and periodic evening or weekend hours for markets, classes, and health fairs. Must be able to sit, stand, walk, and communicate for extended periods; lift and carry event supplies weighing up to 50 pounds; and work outdoors for pop-up market activities in prevailing East Texas weather. This position is funded by the Rural Texas Strong Initiative 1 (Part 1) grant. Continuation of the position is contingent upon availability of grant funds, continued operational need, and satisfactory performance.
