Opportunity Advanced Practitioners - Primary Care Job Details Seeking a full‑time Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join a hybrid remote primary care team Provide Transition of Care, Annual Wellness Visits, and same‑day primary care visits remotely Scheduled on‑site time to support the clinical team and see patients virtually at Campus Care locations Hybrid schedule: virtual/remote from home plus two days in person at Campus Care sites Full‑time weekdays with occasional rotating weekends No call schedule required Work alongside a skilled and flexible team of Advanced Practitioners supporting growing virtual care services Perform routine physical examinations and formulate medical, educational, and psychosocial treatment plans Develop plans for further evaluation of health needs including referrals to specialists and care team members Provide appropriate treatment within scope of practice per established protocols Unrestricted State of Connecticut Medical License required (or eligible) 5+ years of post‑graduate experience as an FNP or PA in a primary care setting required Market‑leading compensation and benefits with generous paid time off Paid CME time off plus CME budget, reimbursed professional licenses and dues Enhanced Tuition Assistance and Higher Education Partnerships available Compensation Employed salary with benefits. #J-18808-Ljbffr BAS HealthCare
