Job Description

Schedule:

• 13-week contract, full-time commitment
• Monday through Friday
• Probable on-call responsibilities
• Structured daily visit expectations

Key Responsibilities:

• Deliver skilled nursing care and interventions in a home setting
• Complete patient assessments and accurately submit OASIS documentation in a timely manner
• Collaborate with physicians and the broader care team to execute individualized care plans
• Perform wound care, medication management, patient education, and disease process monitoring
• Report changes in patient condition and updates to the clinical team promptly

Who We're Looking For:

• Active RN license
• At least one year of recent RN experience in home health
• Proven OASIS documentation experience (required)
• Familiarity with HomeCare HomeBase (strongly preferred)
• Reliable transportation and valid drivers license
• Strong organizational and communication skills to manage a structured, full-time schedule
