Pay Range: $51.53 - $54.11 per hour

Schedule: 40 hours per week (8:00 am - 5:00 pm)

Requirements:

• 2+ years of experience as an RN (Registered Nurse)
• Qualified applicants MUST have at least 2 years of experience in Home Health, HCHB, Oasis, wound care, wound vac, IV, lab draws
• Qualified applicants MUST have Virginia RN license or compact license accepted
• Must have Case Management certification
• Valid Virginia RN license
• Be willing to obtain or maintain state licensure

Responsibilities: As a healthcare professional in a home health setting, perform comprehensive assessments of high-risk members. Collaborate with primary care providers to develop and implement individualized, comprehensive care plans with specific interventions to engage members. Address, identify, and continuously reassess cost-effective, appropriate levels of care. Ensure compliance with treatment plans by coordinating with non-medical support such as housing and transportation. Manage care transitions by collaborating with physicians, social workers, discharge planners, and claims professionals. Engage specialty resources as needed to achieve optimal health outcomes. Keep detailed records of clinical, functional, and fiscal outcomes during the management process. Identify opportunities for health promotion and illness prevention. Act quickly to prevent adverse patient events, acting as a patient advocate by maintaining privacy and confidentiality.
