Hybrid LPN for Community Health & Housing Support

Urban Pathways, Inc. is seeking a Licensed Practical Nurse (LPN) to provide professional nursing care to clients in Bronx, NY. The LPN will assess, develop, and implement nursing care needs while coordinating patient care with external providers. This role offers a hybrid work arrangement of 4 days in office and 1 day remote. Candidates should be graduates of an accredited practical nursing program and preferably have experience with vulnerable populations.
