About the Role:

As a Per Diem LPN, you will be responsible for providing high-quality care to our residents, including administering medications, monitoring vital signs, and assisting with daily living activities. You will work closely with our interdisciplinary care team to ensure that our residents receive the best possible care and services. Your exceptional communication and interpersonal skills will enable you to build strong relationships with our residents, their families, and our staff.

Responsibilities:

• Provide skilled nursing care to residents in a variety of settings, including private rooms, common areas, and rehabilitation spaces
• Administer medications, treatments, and other interventions as prescribed by physicians
• Monitor and report changes in resident condition, health status, and behavior
• Assist with daily living activities, such as bathing, dressing, and grooming
• Develop and maintain strong relationships with residents, families, and staff
• Collaborate with the interdisciplinary care team to develop and implement individualized care plans
• Maintain accurate and detailed records of resident care and services
