Shenandoah Community Health (SCH), a Federally Qualified Health Center and Primary Care Medical Home, is seeking an experienced Family Nurse Practitioner with a minimum of three years of experience to join our Primary Care team in the beautiful Shenandoah Valley. This opportunity offers mission-driven, community-based care with a predictable four- or five-day workweek, no evenings or weekends, low-volume call, and the chance to make a lasting impact in the lives of patients and families.

Our Family Nurse Practitioners provide comprehensive care for patients of all ages, supported by an integrated team that includes behavioral health, pharmacy, care management, interpreters, health educators, and social work resources.

Minimum Qualifications:

• Master's degree or higher from an accredited Nurse Practitioner program
• Current certification as a Family Nurse Practitioner
• Valid license or eligibility for licensure to practice as an Advanced Practice Registered Nurse in the state of WV
• Current WV DEA
• At least three years of Family Nurse Practitioner experience
• All qualifications must be met by start date

Work-Life Balance:

• Flexible four- or five-day workweek
• No evenings, no weekends, and an outpatient-only setting
• Outpatient primary care setting
• Low-volume call rotation every six to eight weeks for one week at a time, with initial calls answered by the answering service

Responsibilities:

• Conduct thorough patient examinations and assessments to diagnose acute and chronic health conditions
• Develop and implement individualized treatment plans based on patient needs and preferences
• Provide preventive care services, including vaccinations, screenings, health education, and chronic disease management
• Collaborate with physicians and other healthcare professionals to coordinate patient care and referrals
• Maintain accurate and up-to-date medical records in compliance with healthcare regulations
