Schedule:

Key Responsibilities:

• Conduct in-home health risk assessments for Medicare Advantage plan members
• Evaluate past and current medical history, medication regimen, and medical conditions
• Educate patients regarding chronic illnesses, conditions, and medications
• Identify care gaps and provide recommendations to address them
• Complete each risk assessment visit (approx. 45 minutes); visits may include annual assessments or post-discharge evaluations

Qualifications:

• Valid Nurse Practitioner license in Virginia
• Board certification from AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• At least 6 months of recent NP experience preferred

This role offers $80-$90 per completed evaluation, plus mileage stipend and compensation for administrative time.
