Seagate Rehabilitation and Nursing Center

Positions:

CNA (All Shifts)

• Pay: Up to $24/hour
• Shifts: 6:00 AM–2:00 PM, 2:00 PM–10:00 PM, 10:00 PM–6:00 AM
• Sign-On Bonus: $1,000 (paid in 4 installments of $250 every 90 days over the first year)

LPN (All Shifts)

• Pay: Up to $33/hour
• Shifts: 6:00 AM–2:00 PM, 2:00 PM–10:00 PM, 10:00 PM–6:00 AM
• Sign-On Bonus: $2,500 (paid in 4 installments of $625 every 90 days over the first year)

RN (All Shifts)

• Pay: Up to $52/hour
• Shifts: 6:00 AM–2:00 PM, 2:00 PM–10:00 PM, 10:00 PM–6:00 AM
• Sign-On Bonus: $5,000 (paid in 4 installments of $1,250 every 90 days over the first year)

RN Supervisor (FT, Evening Shift Only) / RN Unit Manager

• Pay: Up to $110K
• Sign-On Bonus (RN Sup): $7,500 (paid in 4 installments of $1,875 every 90 days over the first year)

Wound Care RN

• Pay: Up to $109K

Location: Seagate Rehab

Onboarding Documents Required:

• Resume
• NYS RN License (RNs)
• NYS LPN License (LPNs)
• NYS CNA Certification (CNAs)
• High school diploma/college diploma/degree certificate (if applicable)
• Valid BLS certificate (nurses only)
• 2 professional reference letters
• Physical performed within the last year, including:
  • PPD result (within last year) or QuantiFERON Gold result
  • MMR, Varicella, Hepatitis B titers report
  • Flu vaccination record
  • COVID-19 vaccination card
• Valid photo ID (passport / permanent resident card / EAD card / driver’s license)
• Copy of authorized Social Security card
• Direct deposit form or voided check

Salary or Hourly Rate

CNA: Up to $24/hour // LPN: Up to $33/hour // RN: Up to $52/hour // RN Supervisor/Unit Manager: Up to $110K
