Responsibilities
- Conduct physical therapy evaluations for residents and establish individualized plans of care.
- Implement therapeutic interventions to improve mobility, strength, balance, and functional ability.
- Reassess progress regularly and modify plans of care to meet goals and outcomes.
- Document treatment sessions clearly in the resident’s health record and communicate with the interdisciplinary team.
- Provide gait training, ADL assistance, and manual therapy as appropriate; monitor safety and fall risk.
- Collaborate with physicians, nursing staff, and family members; participate in discharge planning and ongoing care coordination.

- Availability to participate in a 13-week travel assignment.
- Ability to work effectively in a long-term care/nursing home setting.
- Strong documentation and communication skills for coordination with the care team.
- Proficiency in standard physical therapy modalities and outcome measures.
