In-Home Nurse Practitioner: Geriatrics & Telehealth Assessments

HarmonyCares is seeking a Nurse Practitioner to perform in-home health risk assessments and manage care for patients with complex needs. You will operate within a physician-directed, team-based model and deliver comprehensive visits in home or via telehealth where appropriate. Responsibilities include documenting visits to NCQA standards, communicating with PCPs, caregivers, and nurses, and maintaining licenses and confidentiality while collaborating with the care team. #J-18808-Ljbffr HarmonyCares
