Medical Care Manager

Medical Care Manager Medical Care Manager contributes to AHF by utilizing industry experience, skills, and education while supporting the Ryan White Care Act Program. The role includes client meeting, needs assessment, action/care plans, and ongoing education to ensure patients understand diagnoses and medications. The manager works directly within the Healthcare Center, providing support for the Ryan White program and addressing psychosocial, housing, food, and clothing needs through referrals and assistance. The manager will also facilitate relationships with external programs to support clients lacking services. Responsibilities Conduct needs assessments, develop action/care plans, and provide ongoing patient education. Advise clients on Ryan White eligibility and program resources. Address basic needs such as housing, food, and clothing, and facilitate referrals. Collaborate with providers and staff for optimal care implementation. Track progress toward viral suppression, health outcomes, and quality of life goals. Qualifications BS/BA in Social Work or related social service field with at least two years of direct service delivery. Familiarity with housing case management and housing entitlement systems. Knowledge of HOPWA training modules.
