Client Care Manager

Objective :

The Client Care Manager is responsible for successfully directing and coordinating all activities associated with the care of potential and existing clients. The ideal candidate is organized, professional, compassionate, and understands the importance of teamwork in providing exceptional service to our clients, their families, and our CARE Professionals.

Primary Responsibilities :

• Meet with potential clients and their families for care consultations. At the care consultation, our organization’s services are discussed and the needs of the client are determined so that the Client Care Manager can organize and develop a care plan.
• The CCM manages the entire client experience and process starting from when the client begins service.
• Follow up calls are made to the client and/or their family members periodically to ensure customer satisfaction.
• The CCM meets with all clients within the first 30 days to conduct an initial quality assurance visit.
• The CCM meets with all families and their clients every 90 days to conduct quality assurance visits.
• Develop and maintain a structured, measurable and inclusive Quality Assurance program that builds trust with each client and CAREGiver.

Secondary Responsibilities :

• Maintain and establish strong relationships with the clients, CAREGivers and office staff.
• Answer incoming phone calls in a pleasant, professional, and courteous manner.
• Enter new client information accurately and communicate client schedules and updates with the Scheduler.
• Assist with maintaining and organizing employee and CAREGiver files.
• Assist with filing and processing insurance claims and maintaining related documentation.
• Provide administrative support to the office team as needed.
• CCM has to be willing to fill in as a CAREGiver if needed.

• Strong attention to detail and excellent organizational skills.
• Excellent verbal and written communication skills.
• Professional, friendly, and compassionate telephone manner.
• Ability to multitask, prioritize responsibilities, and meet deadlines.
• Strong computer skills, including proficiency in:
  • Microsoft Word
  • Microsoft Outlook
  • Microsoft Excel
  • WellSky
• Ability to learn new systems and processes quickly.
• Previous administrative, healthcare, home care, or office experience is preferred.
