This position may be eligible for a hiring incentive up to $2,500 as well as relocation reimbursement *** The Community Behavioral Health Hospital (CBHH) in Alexandria is seeking a compassionate, person-centered Licensed Social Worker to join our team dedicated to make a difference in the lives of others. In this key role, you will deliver integrated health care and social services, along with participating as an active member of the interdisciplinary team. Operated by the Minnesota Department of Human Services, the CBHH is a small (16-bed) inpatient psychiatric facility for adults experiencing acute mental health episodes. The length of stay is usually less than 60 days, during which treatment focuses on rapid assessment, stabilization, patient education, and discharge planning involving community support resources, with the overall goal of preventing future episode. Responsibilities Include Meet one-on-one with patients to provide support, psychoeducation, address barriers to discharge, and care planning Co-facilitates group therapy to provide psychoeducation using the Motivational Interviewing and Enhanced Illness Management and Recovery model Active member of the interdisciplinary team identifying treatment needs, goals, and discharge planning Facilitate patient and family education Initiates appropriate referrals for post-discharge follow-up services Maintains appropriate and effective relationships with community providers Minimum Qualifications Bachelor's degree in psychology, #J-18808-Ljbffr Minnesota Direct Care and Treatment
