Inpatient Rehabilitation Case Manager
Full-Time | Monday–Friday | Rotating Weekends
$45–$53/hour

Help Patients Successfully Transition from Hospital to Home

We are seeking an experienced Case Manager to join a collaborative inpatient rehabilitation team. This position is ideal for a healthcare professional with experience supporting patients recovering from stroke, neurological conditions, spinal cord injuries, orthopedic procedures, amputations, traumatic brain injuries, or other complex medical conditions.

You will coordinate each patient’s care from admission through discharge, remove barriers to a safe transition, and help patients and families prepare for the next stage of recovery.

What You’ll Do

• Manage patient care throughout the inpatient rehabilitation stay
• Begin discharge planning upon admission and identify potential barriers early
• Coordinate safe transitions to home, skilled nursing, outpatient therapy, or other appropriate levels of care
• Partner with physicians, rehabilitation nurses, PTs, OTs, SLPs, patients, and families
• Lead interdisciplinary team conferences and communicate changes to the treatment plan
• Monitor patient progress, length of stay, insurance requirements, and discharge readiness
• Complete continued-stay reviews and support authorization and recertification processes
• Coordinate medical equipment, home health services, outpatient care, transportation, and community resources
• Educate patients and families about rehabilitation goals, available resources, and discharge expectations
• Maintain timely, accurate documentation in accordance with payer and regulatory requirements
• Support utilization review, quality improvement, and efforts to prevent avoidable care delays

Qualifications

• Active RN, Social Work, PT, OT, SLP, Rehabilitation Counseling, or other qualifying healthcare license
• Associate degree in Nursing required for RNs; BSN preferred
• Bachelor’s degree required for other eligible disciplines
• Two or more years of inpatient rehabilitation, acute rehabilitation, hospital case management, or complex discharge-planning experience strongly preferred
• Knowledge of post-acute care resources, payer requirements, utilization review, and interdisciplinary care planning
• Ability to manage complex caseloads and multiple discharges in a fast-paced clinical setting
• Eligible to obtain CCM or ACM certification within two years of hire

Pay Details: $26.00 to $46.00 per hour
