- Culminating in thorough patient assessments, ordering relevant diagnostics, and creating tailored care plans
- Diagnosing and addressing acute, ongoing, or chronic health conditions in partnership with physicians or autonomously (based on your credentials)
- Educating patients and their families on preventive care, wellness, and management of illnesses
- Prescribing medications and recommending therapies in accordance with your practice privileges
- Engaging in patient follow-up and chronic care strategies
- Collaborating within interdisciplinary teams to guarantee comprehensive, patient-focused care
- Meticulously documenting all patient interactions and interventions in electronic medical records (EMR)
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Completion of an accredited Nurse Practitioner program
- Active Nurse Practitioner licensure in California
- National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or a related specialty
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the ability to obtain within 90 days)
- Strong clinical judgement complemented by a nurturing approach to patient care
- Outstanding communication skills with patients, their families, and the medical team
- Flexibility in fast-paced, patient-centric environments
- Proficient in EMR systems and comfortable using telehealth technology
- A collaborative spirit and dedication to evidence-based practices
