Job Description

ESI is now hiring a Referral Coordinator in the Stuart, FL area.

POSITION SUMMARY:

ESI is seeking a Referral Coordinator who is responsible for coordinating Client and Care Provider visits, accurately maintaining, updating and scheduling records and logs.

QUALIFICATIONS:

\n
• High school graduate with 2 years of office experience.
\n
• Clear criminal background check
\n
• Must be able to communicate clearly both verbally and in writing
\n
• Understanding Position may be stressful and fast paced
\n
• Primarily a desk job that involves sitting, standing, stooping and walking as well as an inordinate amount of telephone and computer communication
\n
• Ability to walk, stand, stoop, bend, push a cart, or lift during the entire shift
\n
• Ability to lift up to 25 pounds, including small packages and supplies
\n

DUTIES AND RESPONSIBILITIES:

• Coordinate Client and Care Provider visits, accurately maintain and update scheduling records and logs
\n
• Participate and support the recruitment and placement of direct care staff based on Care Provider’s credentials, qualifications, licensure and training
\n
• Support Clients by listening to them and providing clear communication regarding the company's understanding of their needs and requests
\n
• Support Care Providers by listening to them and providing clear communication regarding the company's understanding of their needs and requests
\n
• Prepare calendars based on Client’s needs, requests and Plan of Care matched to the most qualified Care Provider with requested times and skills available
\n
• Monitor and control schedule changes being sure to communicate changes to Client, Care Provider and EPC staff as appropriate
\n
• Ensure appropriate notes are made in Client and Care Provider charts as well as in the Call Center as appropriate
\n
• Provide Care Providers with Plan of Care and calendar, confirm tasks are assigned and care notes are enabled
\n
• Participate in Agency meetings to collaborate and coordinate ongoing needs and responsibilities. Bring PR or coverage issues to supervisor immediately
\n
• Maintain coverage of cases with minimal amount of Care Provider changes
\n
• Maintain and respect privacy of information
\n
• Keep work area clean, tidy and presentable
\n
