We are currently seeking a dedicated and compassionate Physical Therapist (PT) to join a Skilled Nursing Facility team in San Francisco, CA . This full-time opportunity is ideal for professionals passionate about helping patients restore mobility, improve strength, and regain independence through personalized rehabilitation care.

As a PT in the SNF setting, you will work with patients recovering from surgery, illness, injury, or age-related conditions while collaborating closely with interdisciplinary healthcare teams to support successful outcomes and improve quality of life.

Responsibilities:

• Evaluate patients’ physical abilities, functional limitations, and rehabilitation needs
• Develop and implement individualized treatment plans focused on mobility, balance, strength, and independence
• Provide therapeutic interventions and monitor patient progress toward goals
• Educate patients and caregivers on exercises, mobility strategies, and injury prevention techniques
• Maintain timely and accurate documentation in compliance with facility guidelines
• Collaborate with nursing staff, therapists, physicians, and interdisciplinary teams
• Participate in discharge planning and recommendations for continued patient care

Requirements

Qualifications:

• Active Physical Therapist (PT) license required
• Previous SNF experience preferred , but qualified candidates with related experience are encouraged to apply
• Strong clinical reasoning and patient assessment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and within a collaborative team environment
