Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Day Hours per Shift: 36 Experience: Minimum 1 year License: Current PT License Certifications: BLS, Driver’s License, Auto Insurance, Auto Registration, TB test (annual), MMR, Varicella, Hepatitis B, TDaP (10 years), Flu (annual), Drug screen, Physical Must-Have: Willingness to travel outside the area and work weekends if required. Description: We are looking for a qualified Physical Therapist. The assigned county territory includes multiple areas that may require travel. Weekend coverage may be required. If guaranteed hours are applicable, productivity expectations include completing at least 27 points per week. Travelers must complete 27 points in the week to maintain guaranteed hours. Refusing patients may affect guaranteed hours. Guarantee does not apply for the first and last week of the assignment, holidays, or weeks in which the traveler has time off. If no guaranteed hours are applicable, travelers may occasionally need to go outside the service area to secure a full caseload. Travelers can refuse patients. No guaranteed hours are included in this case. Travelers must be willing to see patients with COVID-19 or those suspected of having COVID-19. Payment will be based on the number of visits completed. Visit types will convert to point values that determine approved hours for each day. OASIS re-certification visits, routine visits, evaluations, discharges, and other assessment-related visits will have specific point and hour conversions. All calculated hours include time for drive/travel, visit time, documentation, and phone calls to patients and doctors. Compliance with required credentials is essential for this position. All requested time off must be disclosed during the application process. Any changes or additions to time off requests after submission may not be accepted.
