Registered Nurse Case Manager

Role: Pediatric Case Manager Responsibilities: Support patients and families through their care journey by coordinating services, developing individualized treatment plans, and facilitating communication among care teams. Guide families on program orientation, discharge planning, and insurance benefits. Collaborate with interdisciplinary teams to ensure seamless, patient-centered care, promote positive interactions, and uphold quality standards. Qualifications & Skills: Registered Nurse (RN) required; Bachelor's degree preferred. Minimum of 2+ years in Case Management or 3+ years clinical nursing experience. Familiarity with psychosocial assessments, discharge planning, and InterQual preferred. Strong communication, coordination, and compassionate patient interaction skills. Key Duties: Educate and support patients/families, develop and implement care plans, conduct assessments, document findings, and ensure safe care transitions.
