Case Manager

Description Job Title: Community Case Manager FLSA Status: Non-Exempt Summary: Provides high quality individual and family support for adolescents and their families. Supports clinical decision-making in a variety of agency settings. Provides office based, community, and in-home interventions as an essential job function. Completes necessary documentation of assessment of need, planning and treatment activity. Exhibits concern/empathy for clients and their life experiences. Ability to work autonomously and identify/communicate issues to clinical supervisor. Major Responsibilities / Activities Provides individual and family support to young people and their families. Assesses client and family needs to develop individualized goals in a treatment plan. Provides community based and in-home interventions. Fully documents initial and ongoing assessment of need, individual treatment plans and case notes according to agency and professional standards. Engages in crisis planning with youth and their families. Makes recommendations relative to planning for client discharge. Makes referrals and linkage to other community services. Functions as an advocate for youth and families with Children Services, the county court, and other relative agencies. Provides consultation on cases to agency staff as needed. Maintains required caseload and/or meets established productivity standards. Supports targeted outreach and engagement activities to increase awareness of services and resources. Participates in ongoing trainings and supervision meetings. Accurately completes documentation in a timely manner. Serves on an agency committee. Completes other duties as assigned. Working Conditions and Physical Requirements Work locations may be in a shelter/office environment and/or various locations in the community. Must be able to sit, stand, climb stairs, and use a computer and phone for extended periods. Must drive personal vehicle in order to accomplish work objectives. Nonstandard workweek with some evenings and weekend hours. Must participate in on-call rotation, providing staff support outside of regular working hours May provide coverage for position, functions or shift needs. Requirements Education, Credentials and Qualifications High School diploma or GED required; Bachelor’s degree (B.A/B.S.) in social work or related degree preferred. Related experience or training required with 2 years of experience preferred; or an equivalent combination of education and experience. Strong verbal and written communication skills. Computer literate with strong data entry skills. Ability to relate to and have empathy for young people. Holds and maintains a valid driver’s license with a good driving record. #J-18808-Ljbffr
