Nurse PractitionerJoin Carease Health - IL, a nationwide preventative healthcare company, in our mission to reduce hospitalizations and improve the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care.About the RoleWe are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities.
