Coverage Territory: Bloomington, Pontiac, Streator, Seneca, Ottawa, LaSalle Peru, Dwight, Marseilles, and surrounding areas. What You'll Do As a Transitions Hospice Night Nurse, you'll serve as a critical support member of the interdisciplinary care team during overnight hours — assessing, treating, and communicating patient needs with clinical confidence and genuine compassion. Clinical experience in hospice, home health, or a related setting preferred Comfort and composure working independently during overnight hours Strong assessment skills and sound clinical judgment in high-pressure moments A calm, reassuring presence for patients and families during nighttime distress Excellent documentation habits and communication skills Valid driver's license and willingness to travel locally
