Licensed Practical Nurse - LTC

Job Opportunity
The position will require availability during the following hours:
- Thursdays & Fridays: 6 PM to 6:30 AM
- Saturdays & Sundays: 10 PM to 6:30 AM
Role Responsibilities
- Provide comprehensive care to patients, ensuring their needs are met with empathy and professionalism.
- Collaborate with interdisciplinary teams to develop and implement effective patient care plans.
- Monitor patient progress and make necessary adjustments to care plans as needed.
- Document all patient interactions accurately and in a timely manner.
- Adhere to established policies and procedures to maintain a safe and compliant environment.
Qualifications
- Must possess relevant licensure and certifications applicable to the role.
- Strong clinical skills and ability to work in a fast-paced environment.
- Excellent communication and interpersonal skills.
- Ability to collaborate effectively with a diverse team of healthcare professionals.
- Prior experience in a similar role is preferred.
