Hospice Registered Nurse (RN) - Full-Time | Up to $95,000 | 12-Hour Shifts

JourneyCare Hospice is seeking a dedicated Hospice Registered Nurse (RN) to provide compassionate, patient-centered care in the South Chicago area. This is an excellent opportunity for nurses with hospice, home health, or acute care experience looking to make a meaningful impact.

We are offering competitive pay up to $95,000 annually , a supportive team environment, and strong benefits starting quickly.

Location: South Chicago (Beverly and surrounding areas), IL
Schedule: Full-Time | Three 12-hour shifts (10:00 AM - 10:30 PM)
Includes two weekend shifts per month + one major and one minor holiday annually

Key Responsibilities

• Perform hospice admissions and pre-admission assessments
• Educate patients and families on hospice care and services
• Develop and implement individualized care plans
• Provide hands-on nursing care focused on comfort and quality of life
• Collaborate with interdisciplinary care teams
• Ensure compliance with all clinical and quality standards

Qualifications

• Active RN license in Illinois
• Graduate of an accredited nursing program
• Current CPR certification
• Minimum 1 year of hospice experience preferred (home health or med-surg considered)
• Strong assessment and critical thinking skills
• Ability to work independently in the field
• Excellent communication and patient care skills
